             Independent Inventor - Instructions for using this form: Press the "insert" key  prior to adding data.  To change the default check fields, go in the tool dialog box and click on the "Protect document".   In this box click the button beside "Forms" and then click on "OK".  The tool dialog box changes to "Unprotect document".    Make changes to the appropriate check field, and then change the tool dialog box back to "Protect document".  Remember you can only change a field when the tool dialog box says "Unprotect document", and you can only insert type when the tool dialog box says "Protect document".
